Many of the errors result from unclear abbreviations and dosage indications, and illegible writing on some of the 3.2 billion prescriptions written in the United states. In 1999, an American cardiologist caused the death of a 42-year-old patient when his prescription of 20mg Isordil, an antianginal drug was misread as plendil, an antihypentensive drug.
